ABSTRACT

A cross-sectional study was conducted to estimate the age-stratified normal levels and age-related changes in the risk predictors of benign prostatic hyperplasia (BPH) progression. A total of 4706 male participants aged 40 years or older in Zhengzhou (China) were enrolled. The values of the International Prostate Symptom Score (IPSS), prostate-specific antigen (PSA), prostate volume (PV), and postvoid residual urine volume (PVR) significantly increased with age. Nonlinear relationships between age and IPSS scores ≥8 (P for nonlinearity = 0.046), PSA level ≥1.6 ng ml-1, PV ≥31 ml, or PVR ≥39 ml (all P for nonlinearity <0.001) were observed. After the age of 61 years, the risk indicators related to BPH progression were positively correlated with age (odds ratio [OR] >1), regardless of the predictors of the IPSS score, PSA level, PV, or PVR; and the OR values increased gradually. Therefore, after the age of 61 years, the risk predictors related to BPH progression were positively correlated with age.

ABSTRACT

OBJECTIVE@#To observe the effect of San-Ao Decoction (, SAD) on water metabolism of bronchial asthra model mice.@*METHODS@#Forty-five female BALB/c mice were randomly divided into control, model and SAD groups by a random number table, 15 mice in each group. A composite method with ovalbumin (OVA) sensitization and challenge was developed to establish bronchial asthma model. Mice in the control group were intraperitoneally injected with distilled water without aerosol inhalation challenge. On day 15-22, 0.3 mL SAD was administered via gastric route in SAD group, one time per day, while an equivalent volume of normal saline was used for gastric administration in the control and model groups. Changes in airway resistance in the inspiratory phase (RI-R-Area) were detected using an AniRes2005 system, and 5-h urine output was collected by metabolic cages. Histopathological changes in lung and kidney were observed by hematoxylin-eosin staining. mRNA expressions of aquaporin (AQP) 1 and AQP2 in kidney were detected by reverse transcription-polymerase chain reaction, and the protein expressions of AQP1 and AQP2 in kidney were detected by immunohistochemistry. Enzyme-linked immune sorbent assay was used to detect the OVA-specific endothelium-1 (ET-1), antidiuretic hormone (ADH), atrial natriuretic peptide (ANP), prostaglandin E@*RESULTS@#Compared with the control group, the serum IgE level in model group increased (P<0.01). Following the pathologic changes in lung tissue, no significant change in kidney tissue was observed among 3 groups. Compared with the control group, the mice in the model group showed elevated airway resistance during inhalation phase, higher mRNA and protein expression levels on AQP1 and AQP2 in kidney tissue and higher ET-1 levels in serum, lung and kidney tissues, ADH and ANP in lung and serum, PGE@*CONCLUSION@#San-Ao Decoction can regulate the urine volume through regulating AQP1 and AQP2 expression, and the expression of these in the kidneys might be regulated by ET-1, NO and Ang II.

@#Objective To analyze urine volume and dietary salt intake assessed by two 24 h urine specimens,to discuss its application value for evaluating population level and individual level dietary salt intake. Methods The subjects aged 18-69 years were selected from four counties in Shandong Province and Jiangsu Province from 2013 to 2014 by using multi-stage stratified cluster random sampling method. A questionnaire survey was conducted and two 24 h urine with an interval of one day were collected. The differences between two 24 h urine volume,and the dietary salt intake between two 24 h urine were assessed at individual level and population level. Results A total of 1 288 subjects was( 42.3± 14.0) years old,of which 626( 48.6%) were males. The average urine volume of the subjects was ( 1 462±437) ml. The first 24 h urine volume ( 1 427±488) ml was lower than the second 24 h urine volume ( 1 498± 552) ml ( t = -4.439,P<0.001) . The dietary salt intake was ( 9.8±3.3) g,and the dietary salt intake of males ( 10.1±3.5) g was higher than that of females ( 9.5±3.1) g ( t = 3.09,P= 0.002) . There was a significant difference in dietary salt intake among different age groups ( F= 7.57,P<0.001) . The dietary saltintake of 1 136 ( 88.2%) subjects was higher than the recommended level. At the individual level,the participants with the absolute difference with 1g between the subjects’two 24 h urinary salt excretion was 279 ( 21.7%) ,and the participants with the absolute difference with >3 g was 48.5%. At the population level,the first 24 h urinary salt excretions ( 9.9 ± 4.1) g was similar to that in the second 24 h urine and ( 9.7±4.0) g ( P= 0.1021) . The intraclass correlation coefficients between salt excretions in the two 24 h urine was 0.508( 95% CI: 0.451-0.559) . Conclusion The results of this study suggest that 24 h urinary salt excretion can better assess the salt intake at population level,but cannot accurately reflect the individual’s salt intake.

Objective: To investigate the diuretic and renal effects of Silybum marianum L. and Cistus ladaniferus L. in normal rats. Methods: Four groups of rats were used in each experiment. On the day 8 after daily administration, the plant extracts induced a significant diuresis and natriuresis without affecting serum electrolytes (P<0.05), while furosemide caused hypokalemia. Both plant extracts significantly increased creatinine clearance (P<0.05). Conclusions: Silybum marianum L. and Cistus ladaniferus L. increase creatinine clearance and have a significant diuretic effect without affecting serum electrolytes. ABSTRACT

Objective To investigate the clinical efficacy of acupuncture plus pelvic floor biofeedback treatment for the neurogenic bladder following spinal cord injury.Method Ninety patients with neurogenic bladder following spinal cord injury were randomly allocated to acupuncture, biofeedback and combination groups, 30 cases each. Every group of patients received acupuncture, pelvic floor biofeedback treatment or a combination of two treatments in addition to conventional bladder rehabilitation training. The LUTS score and the quality of life score were recorded and the bladder diary was kept before and after treatment. A statistical analysis was carried out.Result The LUTS score, the quality of life score, the residual urine volume and the mean voiding frequency decreased significantly in the three groups of patients after four weeks of treatment compared with before treatment (P<0.05) and decreased more significantly in the combination group than in the acupuncture and biofeedback groups (P<0.05). Maximum voided volume increased significantly in the three groups of patients after treatment compared with before (P<0.05) and increased more significantly in the combination group than in the other two groups (P<0.05).Conclusion Acupuncture plus pelvic floor biofeedback can significantly improve bladder function and the quality of life in patients with neurogenic bladder following spinal cord injury. Its clinical therapeutic effect is superior to that of acupuncture or biofeedback.

ABSTRACT

Objective To assess the clinical therapeutic effect of electroacupuncture on urinary retention due to prostatic hyperplasia by ultrasonic measurement of bladder residual urine volume before and after treatment.Methods Seventy patients with urinary retention due to prostatic hyperplasia were randomized to a treatment group (39 cases) and a control group (31 cases).The treatment group received electroacupuncture and the control group,conventional medication.The International Prostate Symptom Score (I-PSS) score was recorded and prostate volume (PV) and bladder residual urine volume (RUV) were measured in the two groups before and after treatment.The clinical therapeutic effects were compared between the two groups.Results There were statistically significant pre-/post-treatment differences in the indicators (I-PSS,PV and RUV) in both groups (P<0.05,P<0.01).There were statistically significant post-treatment differences in the indicators between the treatment and control groups (P<0.05).The total efficacy rate was 94.9% in the treatment group and 96.8% in the control group;there was no statistically significant difference between the two groups (P>0.05).Conclusion Both electroacupuncture and medication are effective ways to treat urinary retention due to prostatic hyperplasia.

ABSTRACT

Objective To evaluate the effect of hypothermia status in the donors upon the renal graft function after renal transplantation from donation after citizen's death. Methods Thirty-six eligible donors were randomly divided into the normal temperature (body temperature 36.5-37.5 ℃ , n=19) and hypothermia groups (body temperature 34.0-35.0 ℃ , n=17). The matched recipients undergoing renal transplantation were also assigned into the normal temperature (n=38) and hypothermia groups (n=34). Perioperative conditions of the donors and recipients were compared between two groups. And postoperative renal graft function of the recipients were statistically compared between two groups, including the incidence of delayed graft function (DGF) and primary nonfunction (PNF). Results No statistical significance was identified in the perioperative amount of urine volume, serum creatinine (Scr), systolic blood pressure, saturation oxygen, warm ischemia time and cold ischemia time of the donors between two groups (all P>0.05). No statistical significance was noted in terms of the operation time, intraoperative mean blood glucose and intraoperative mean arterial pressure of the recipients between two groups (all P>0.05). Postoperative incidence of DGF of the recipients in the hypothermia group was 6%, significantly lower than that in the normal temperature group (24%) (χ2=4.393, P=0.036). Postoperative incidence of PNF of the recipients was 3% in both the hypothermia and normal temperature groups with no statistical significance (χ2=0.000, P=1). Conclusions The hypothermia status of the donors can significantly reduce the incidence of DGF, whereas exerts no evident effect upon the incidence of PNF in the recipients.

La enuresis primaria monosintomática es un problema clínico común que afecta del 5 al 10% de los niños en edad escolar. La etiología exacta no se conoce, pero se ha propuesto una capacidad vesical funcional disminuida como factor predisponente. Existen pocos reportes en la literatura del uso de ultrasonido para su evaluación. Objetivo: Determinar si la capacidad vesical funcional medida por ultrasonido está disminuida en enuresis primaria monosintomática comparada con población sana. Pacientes y Método: Estudio trasversal analítico de febrero de 2014 a mayo de 2015 incluyendo 40 pacientes con enuresis y 40 sin enuresis, 5 a 15 años de edad, midiendo la capacidad vesical funcional mediante ecógrafo Siemens Acuson S2000TM con transductor 3,5 y 5 MHz por un solo operador cegado. Se realizó estadística descriptiva y analítica mediante el programa IBM SPSS 20 TM. Resultados: Los pacientes con enuresis presentaron menor capacidad vesical funcional 171,7 ml vs controles 225,5 ml (p = 0,025). Resultó factor de riesgo OR = 2,81 (IC 95%: 1,06-7,42) tener un familiar de primera línea con antecedente de enuresis y OR = 4,0 (IC 95%: 1,48-10,78) para segunda línea. La capacidad vesical funcional presentó correlación débil con la capacidad vesical normal estimada mediante la fórmula de Kaefer. Conclusión: La capacidad vesical funcional es menor en quien padece enuresis que en los que no la padecen y existe poca correlación con las fórmulas que determinan la capacidad vesical normal esperada como la de Kaefer. Se reafirmó que el antecedente hereditario de enuresis juega un papel importante como factor de riesgo.


Nocturnal enuresis is a common clinical problem affecting 5% to 10% of school-age children. Etiology is not known but a diminished functional bladder capacity it has been proposed as a predisposing factor. There exist only a few studies evaluating it by ultrasound. Objective: To identify if there is a difference of the functional bladder capacity measured by ultrasound between nocturnal enuresis group and healthy children. Patients and Method: A cross-sectional study from February 2014 to May 2015 including two groups, nocturnal enuresis and a control group of 40 patients each, 5 to 15 years old. A single blinded operator measured the functional bladder capacity by ultrasound with an Acuson S2000 SiemensTM 3.5 and 5 MHz transducer. Analytics and descriptive statistics were performed using IBM SPSS 20TM software. Results: Patients with enuresis showed a decreased functional bladder capacity vs. controls (171.7 ml vs 225.5 ml; p = 0.025). A history of first-degree relative with enuresis increased the risk of having enuresis OR = 2.81 (95% CI: 1.06-7.42), a second-degree relative presented OR = 4.0 (95% CI: 1.48-10.78). Functional bladder capacity presented a weak correlation with the bladder capacity estimated by Kaefer’s formula. Conclusion: The functional bladder capacity is lower in the patients with nocturnal enuresis when compared to control group. There is little correlation between functional bladder capacity and Kaefer’s formula to determine the normal bladder capacity. We reaffirmed that the family history with enuresis strongly increases the risk of developing nocturnal enuresis.

ABSTRACT

Objective To observe the therapeutic efficacy of fire-needle acupuncture in treating benign prostatic hyperplasia. Method Sixty subjects were randomized into two groups by using the sealed envelope method. The control group was intervened by conventional medications; after classified into four syndrome types, down diffusion of damp-heat, liver qi stagnation, turbid stagnation and kidney qi deficiency, the treatment group was majorly treated with fire-needle acupuncture in addition to filiform-needle acupuncture. Fire-needle acupuncture was performed at Guanyuan (CV4), Qugu (CV2) and bilateral Dahe (KI12), once a week, for continuous 6 months; the filiform-needle acupuncture was majorly at Baihui (GV20) and Qihai (CV6) with adjunctive points modified according to different syndromes, twice a week, 8 sessions as a treatment course, for 5 courses in total. Result The improved rate was significantly higher in the treatment group than in the control group (P<0.05);the residual urine volume was significantly changed after the intervention in both groups (P<0.01), and the decrease in the treatment group was more significant than that in the control group (P<0.05). Conclusion Fire-needle acupuncture can improve the symptoms of benign prostatic hyperplasia and lower the residual urine volume.

ABSTRACT

OBJECTIVE:To observe clinical efficacy and safety of torasemide in the treatment of chronic heart failure(CHF). METHODS:94 patients with CHF were selected and randomly divided into control group and observation group,with 47 cases in each group. Control group were treated with routine treatment,such as bed rest,oxygen inhalation,control of total body fluid in-take,routine anti-heart failure therapy and use of diuretic in 24 h. Based on the above treatment,observation group were treated with Torasemide injection with initial dose of 5-10 mg,qd,gradually increasing to 20 mg/d,maximal does of 40 mg/d. Both group re-ceived 7 days of continuous treatment. Clinical efficacies were observed in 2 groups as well as serum potassium and sodium,Scr,24 h urine volume and the rate ofⅠ-Ⅱgrade cardiac function before and after treatment. The occurrence of ADR was compared between 2 groups. RESULTS:The total effective rate of observation group was 61.70%,which was significantly higher than that of control group (46.81%),with statistical significance (P0.05). There was no statistical significance in 24 h urine volume and the rate ofⅠ-Ⅱgrade cardiac function between 2 groups before and after treatment(P>0.05);after treatment,24 h urine volume and the rate ofⅠ-Ⅱgrade cardiac function of 2 groups were increased significantly,the observation group was higher than the control group, with statistical significance (P0.05). CONCLUSIONS:Torasemide is effective for CHF,and can promote diuresis and the recovery of cardiac function with good safety.

ABSTRACT

Objective To assess the effectiveness and safety of dexmedetomidine (Dex) in anesthesia for end-stage renal disease (ESRD)recipients in living related renal transplantation.Methods Forty ESRD patients undergoing living related renal transplantation in the Affiliated Provincial Hospital of Anhui Medical University from October 201 3 to December 201 4 were randomized into the Dex group and the control group,20 patients in each group.In the Dex group,the patients were pumped with the loading dose of Dex at 0.6 μg/kg before anesthesia induction and the procedure was completed within 20 min.Then,the patients were pumped at 0.2 μg/(kg·h)for 1 h and underwent general anesthesia induction.In the control group,the patients were pumped with equivalent normal saline.The anesthesia induction and the maintenance drug of the two groups were the same.The mean arterial pressure (MAP),heart rate (HR)and bispectral index (BIS) before administration (T0 ),20 min after administration (T1 ),before laryngeal mask placement (T2 )and after laryngeal mask placement (T3 )of the two groups were observed and recorded.Delayed recovery and emergence delirium were also observed.Urine output during transplantation,4,8,1 2,24 h after transplantation,as well as endogenous creatinine clearance rate (Ccr)before transplantation and 1 2,24 h after transplantation were recorded.Results Compared with T0 ,the MAP,HR and BIS of the two groups at T2 decreased significantly (all in P <0.05).As for the comparison between two groups,the HR and BIS of the Dex group decreased significantly (both in P <0.05 ),but the MAP didn't decrease significantly (P >0.05 ).The emergence delirium in the Dex group was all mild with the incidence of 1 5% (3 /20),which significantly deceased in comparison with that in the control group (30%,6 /20)(P <0.05).The urine outputs of Dex group during transplantation,and 4 h and 8 h after transplantation were significantly higher than those of the control group (all in P <0.05).The Ccr of the two groups at 1 2 h and 24 h after transplantation significantly increased, compared with that before transplantation (all in P <0.01 ).Conclusions Dex may reduce the incidence of emergence delirium of recipient in living related renal transplantation,increase urine output after transplantation and cause no delayed recovery,which may be used in ESRD patients safely.

Objective To observe the clinical efficacy of acupuncture plus moxibustion in the treatment of benign prostatic hyperplasia.Method Sixty cases of benign prostatic hyperplasia were randomly divided into two groups, 30 cases in the treatment group, 30 cases in the control group. The treatment group was intervened by Acupuncture and moxibustion, while the control group was by orally takingJingzhu Qianlie Longbitong capsules. The International Prostate Symptom Score (IPSS), Quality of Life score (QOL), maximal urinary flow rate (Qmax), residual urine volume (RUV) were compared before and after 3 courses of treatment. Result The total effective rate was 93.3% in the treatment group, versus 66.7% in the control group, and the difference was statistically significant (P<0.05); the IPSS score, QOL score, Qmax, and RUV were significantly changed in both groups after intervention (P<0.05).Conclusion The treatment group and the control group both show improvements in IPSS, QOL score and Qmax and reduction of RUV, which proves that acupuncture plus moxibustion can produce a content efficacy in treating benign prostatic hyperplasia.

ABSTRACT

Objective To evaluate the effectiveness of mild hypothermia and the intra-aortic balloon pump (IABP) in postcardiac surgical patients with severe heart failure. Methods Twelve patients took combination therapy of IABP and mild hypothermia after cardiac surgery, which did not improve the low cardiac output with massive doses of catecholamine, were included in this study. The values of cardiac index (CI), mixed venous oxygen saturation (SvO2), urine volume and creati-nine (Cr) were measured before and after treatment. Results After combination therapy of IABP and mild hypothermia, the value of CI was improved obviously [L/(min·m2):2.4±0.5 vs 1.6±0.2], the value of SvO2 increased (0.65±0.07 vs 0.52±0.05 ), urine output increased obviously [mL/(kg·h):2.9±1.9 vs 1.2±0.9 ] and the value of Cr decreased obviously (mmol/L:95±10 vs 282±25 ). Conclusion The combination therapy of mild hypothermia and IABP is an effective and simple procedure for the postcardiac surgical patients with severe heart failure.

ABSTRACT

Objective To explore the effect of modified Zexie Decoction on renal aquaporin-2 (AQP2) expression in high-salt hypertensive rat. Methods Hypertensive rats model was established by feeding rat with 8%high salt. Rats (n=50) were divided into model group, modern medicine group, traditional Chinese medicine groups of high, medium, low dose, with 10 rats in each group. The other 10 rats were fed with ordinary diet as normal group. Rats in traditional Chinese medi?cine of high, medium, low groups were given Zexie Decoction suspension of 16.2, 10.8 and 5.4 g/(kg·d) respectively;Rats in modern medicine group was given Valsartan hydrochlorothiazide 16.65 mg/(kg·d);the model group and normal group was ad?ministered with equal volume of distilled water. Animals were feed with medications at 1 mL/100 g by gavage for 4 weeks. On the 1st , 4th , 7th , 14th and, 28th day of administration, we measured SBP and collected 24 h urine. We employed immunohis?tochemistry to detect renal AQP-2 protein expression level and RT-PCR to detect renal AQP-2 mRNA transcription level. Results The rank of SBP from high to low is:model group>traditional Chinese medicine medium and low dose groups>traditional Chinese medicine high dose group and western medicine group>normal group. The rank of urine volume from high to low is:Western medicine group and traditional Chinese medicine high dose group>traditional Chinese medicine me?dium and low dose group > normal group, the difference was not statistically significant between traditional Chinese medi?cine medium and low dose group , or between western medicine group and traditional Chinese medicine high dose group. The renal AQP-2 in epithelial cells along the collecting duct wall of rats in model group show brown particles which are darker and wider distributed than those in normal group and traditional Chinese medicine of high, medium, low dose groups. RT-PCR results show that AQP-2 mRNA expression is highest in rats of model group and lowest in rats of traditional Chi?nese medicine high dose group (P<0.05). No statistical significance of AQP mRNA level was found between traditional Chi?nese medicine low group and model group (P < 0.05). Conclusion Modified Zexie Decoction can lower blood pres?sure by inhibiting the expression of AQP-2.

ABSTRACT

Urinary excretion is fundamental for the understanding of several variables related to ruminant nutrition. The total collection of urine from grazing animals is a complex and difficult procedure, while in confined animal it requires the installation of funnels or urinary catheters that generate discomfort along with an increased risk of injury. Objective: the aim of this study was to validate the predictive ability of urine creatinine on urine output, in comparison with the total collection method. Methods: five Nelore animals were used. Animals were kept in confinement conditions and were offered feed and water ad libitum. The total volume of urine produced, determined by total collection in metabolic cages was compared with the estimated production calculated from creatinine excretion. Spot samples collected four hours after feeding the animals were analyzed for creatinine concentration during four experimental periods. Results: the statistical significance of paired t-test and the trend of the regression slope in the Bland-Altman analysis allow concluding that creatinine excretion, under our experimental conditions, was not an adequate predictor of urine volume. Conclusions: creatinine concentration in spot samples can be considered as the critical factor in this methodology due to the common diurnal variations of urine output and, therefore, the variations in the excretion of the metabolite.


El conocimiento de la excreción urinaria es necesario para la determinación de diversas variables contempladas en la nutrición de rumiantes. Con animales en pastoreo, la recolección total de la orina representa un procedimiento complejo; y en confinamiento, demanda la instalación de embudos o sondas urinarias, que generan incomodidad y aumentan el riesgo de lesiones. Objetivo: el objetivo de este trabajo fue validar la capacidad de predicción de la producción urinaria a través de la excreción de creatinina, con respecto a la determinación realizada vía recolección total. Métodos: cinco cebuínos criados en régimen de confinamiento, recibiendo alimento y agua ad libitum, fueron empleados. La producción total de orina, determinada mediante recolección total en jaula metabólica, fue comparada con la producción estimada desde la excreción de creatinina. Muestras spot fueron recolectadas cuatro horas después de la alimentación y analizadas para concentración de creatinina durante cuatro fases experimentales. Resultados: la significancia estadística de la prueba t pareada y a la tendencia registrada por la pendiente de regresión en el análisis de Bland-Altman, permiten concluir que la excreción de creatinina, bajo nuestras condiciones experimentales, no predijo adecuadamente el volumen urinario. Conclusiones: la concentración de creatinina en la muestra spot se considera el punto crítico de la metodología por las variaciones diurnas que se registran en la producción urinaria y, por tanto, en la excreción del metabolito.


O conhecimento da excreção urinária é necessário para o estabelecimento de diversas variáveis listadas na nutrição de ruminantes. Com animais em pastejo, a coleta total de urina é um procedimento complexo, e em confinamento, requer a instalação de funis ou de cateteres urinários, o que gera desconforto e aumenta o risco de lesões. Objetivo: o objetivo deste estudo foi validar a capacidade preditiva da produção urinária através da excreção de creatinina, respeito a determinação feita por meio da coleta total. Métodos: cinco animais zebus criados em confinamento, recebendo alimento e água ad libitum, foram usados. A produção total de urina, medida pela coleta total em gaiolas metabólicas, foi comparada com a produção estimada desde a excreção de creatinina. Amostras spot foram coletadas quatro horas após alimentação e analisadas para concentração de creatinina durante quatro fases experimentais. ABSTRACT

Objective To access the effect of intermittent catheterization (IC) on the urinary tract infection in patients with spinal cord injury. Methods Sixty patients of spinal cord injury combined with urinary tract infec-tion were randomly divided into two groups: treatment group (32 patients) treated with sensitive antibiotics and IC;control group (28 patients) treated with sensitive antibiotics only. Both groups were treated for two weeks. Results In the treatment and control group,the cure rate was 56% and 29% ,respectively,the total effective rate was 91% and 64% in the treatment and control group,respectively,the clearance rate of bacteria was 81% and 57% ,respec-tively. All differences were significant (P<0.05). Conclusions IC can improve the effect of antibiotic on urinary tract infection in patients with spinal cord injury, and might be effective on the treatment of urinary tract infection caused by various neurogenic bladders.

ABSTRACT

Objective To discuss the correlation between residual urine volume after catheterization and body positions for patients with spinal cord injury using B-ultrasonography. Methods 34 patients with spinal cord injury were randomly selected, the residual urine volume in urinary bladder was detected with bed-side B-ultrasonography under different body positions such as supine position, lateral position and fowler position, the results underwent variance analysis. Results No significant difference was seen in residual urine volume in urinary bladder under different body positions. Conclusions Body positions play no significant influence on residual urine volume in urinary bladder after catheterization.

ABSTRACT

PURPOSE: We assessed the accuracy of a portable ultrasound device (BladderScan(TM) BVI 3000) used to measure the postvoid residual urine volume (PVR). MATERIALS AND METHODS: We prospectively measured the volume of residual urine in 160 patients with lower urinary tract symptoms between August 2000 and April 2001. After voiding, PVR measurements were performed immediately using ultrasound bladder scanning, followed by Nelaton catheterization, with the patient was in the supine position, and compared the results of the two techniques. RESULTS: The ultrasound bladder scanning correlated well with the catheterization for measuring the PVR (r=0.946). The ultrasound bladder scanning had a sensitivity and specificity of 95.4 and 100%, respectively, in detecting a PVR>or=100ml, and 83.4 and 91.5%, respectively, in cases where the PVR was less than 50ml. A multiple regression analysis, and a two sample t-test, showed that the difference was not related to: age, sex, weight, height, body mass index (BMI) or related diseases (p>0.05). Age, weight, height, BMI were not significant variables when comparing the sexes (p>0.05). There was a significant difference in patient's satisfaction and the required time of the procedure between bladder scanning and catheterization (p<0.01). CONCLUSIONS: Portable ultrasound bladder scanning is quick, easy to use, non-invasive, readily repeatable, and specific for determining the PVR. Therefore, portable ultrasound bladder scanning can be used as an alternative to catheterization in the determination of residual urine volume.

ABSTRACT

This study was conducted to compare the efficacy of Terazosin given in alternate day doses with that of the same drug given in daily doses in the medical management of benign prostatic hyperplasia (BPH) using the following parameters: International Prostate Symptom Score (IPSS), urine flow rate, residual urine volume and quality of life. Ninety patients were randomly assigned to Daily Dose, Alternate Day Dose and Placebo groups. Symptom scores using the IPSS were measured at pre-treatment, and at 1st, 3rd and 6th week treatment periods. Pre- and post-treatment measurements of urine flow rate, residual urine volume and quality of life were done. Two-way ANOVA and Wilcoxon Matched Pair Signed Ranked Test was used to determine the level of significance. The study showed that there was significant improvement in the symptom score in both Daily Dose (15.80 +/- 1.74 to 5.93 +/- 1.03) (p 0.001) and Alternate Day Dose group (16.06 +/- 1.87 to 6.13 +/- 1.45) (p 0.001) compared to the placenta group. Likewise there was significant improvement in the urine flow rate in both groups, (4.82 +/- 0.75 to 5.76 +/- 0.67) (p (65.66 +/- 17.54 to 46.46 +/- 12.77) (p = 0.001) in the Alternate Day Dose group. The quality of life improved (3.26 +/- 0.88 to 1.66 +/- 0.72) (p = 0.002) in the Daily Dose group and (3.66 +/- 0.81 to 1.80 +/- 0.77) (p = 0.001) in the Alternate Day Dose group. (Author)
